Store the observation at the right grain
One ledger entry should represent one distinct need, friction point, request, or positive outcome. Preserve the customer wording when permitted, but remove unnecessary personal data and link to the controlled source record.
Code themes without erasing nuance
A theme tag helps aggregation, while context fields explain the workflow, user type, product area, and consequence. Keep the raw observation separate from the account team interpretation so later reviewers can challenge the coding.
Close the feedback loop honestly
Record whether feedback was acknowledged, routed, evaluated, planned, declined, or still open. Do not imply a roadmap commitment. Customer-facing follow-up should use language approved by the responsible product or service owner.
