Keep a Customer Decision History That Survives Handoffs

Write the decision as a complete statement

Record the chosen option, decision owner, effective date, scope, and source. A note such as discussed pricing is not a decision. Include options explicitly rejected only when the record supports that conclusion.

Preserve conditions and tradeoffs

A decision often depends on budget, timeline, staffing, security, or product constraints. Capture those conditions so a future account manager can distinguish an enduring preference from a temporary compromise.

Link later changes instead of overwriting

When a decision changes, create a new entry that references the earlier one. The resulting chain shows why the team acted as it did and prevents an outdated screenshot from becoming the apparent source of truth.

Implementation checklist

  • Name the decision owner
  • State scope and date
  • Record constraints
  • Link source evidence
  • Append revisions
